THIS NOTICE DESCRIBES HOW CHIROPRACTIC AND MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.

The Health Insurance Portability and Accountability Act of 1996 (HIPAA)

HIPAA is a federal law that requires all medical records and other individually identifiable health information—whether written, electronic, or verbal—to be kept strictly confidential.

As part of your care at the office of Gordon Fimreite, DC LLC, we may need to use or disclose your personal and health-related information in the following ways:

• Your health information, including clinical records, may be shared with another healthcare provider or hospital if a referral is needed for diagnosis, assessment, or treatment.
• Your health and billing records may be disclosed to third parties such as insurance companies, HMOs, PPOs, or your employer if they are responsible for payment.
• Your name, address, phone number, and health records may be used to contact you regarding appointment reminders, updates on your care, or to share information that may benefit your health. This may include newsletters, birthday cards, thank-you notes, or referral materials. If you are not home, we may leave a message on your voicemail or with a household member.
  

You have the right to inspect or receive a copy of your health information and to request how and where we contact you. Refusing authorization for these communications will not affect your care or your insurance coverage.

Disclosures Without Consent
HIPAA permits us to disclose your health information without your written consent in certain circumstances, including:

• When services are provided by the order of another healthcare provider.
• In the event of an emergency.
• When care is required by law and we are unable to obtain your consent.
• If there are substantial communication barriers and we determine it is in your best interest to receive care.
• When ordered by a court or legal authority.

All other uses and disclosures will require your written permission.

Communication Preferences
We will normally send billing and health information to your home address. If you prefer to be contacted in another way, please let us know in writing.

Access to Your Records
You have the right to:

• Inspect and/or request a copy of your health records created within the past seven years.
• Request an amendment to your health records.
• Request how and with whom we communicate your information.

All requests must be submitted in writing.

Our Privacy Commitment
We are legally required to protect your health information and to provide this notice of our privacy practices. We must follow the terms described in this notice and will notify you in writing if any changes are made.
Please note: Any information shared under this notice may be re-disclosed by the person who receives it and may no longer be protected under federal law.

Open Concept Practice and Family Involvement

• Open Adjusting Rooms: Our clinic uses an open, warm, family-friendly environment. While private medical details are not discussed in open areas, others may be present. You may request a private consultation if desired.
• Family Involvement: We encourage spouses or family members to attend health report visits unless you request otherwise.

Restrictions and Requests

• You may request specific restrictions on how your information is used or disclosed. While we are not required to agree, we will consider all requests seriously.
• You may request a copy of your records. A fee may apply for printing, mailing, or handling. We will notify you in writing of any charges before proceeding.

Denial of Access
In some situations, we may deny your request to view or copy records. If this happens, you may request a review of the decision by a licensed health professional not involved in the original denial. We will comply with the outcome of that review.
