Policies & Guidelines for Shockwave Therapy
Policies & Guidelines:
- Discounts on Packages:
- Discounts on the 3-visit and 6-visit packages apply only when the full package is purchased in advance.
- The discount is applied to the final visit of the package.
- Refunds for Early Termination:
- If you decide not to complete your package and wish to end it early, you must notify us in writing before the expiration date.
- In such cases, we will refund the remaining balance of the package you’ve paid.
- If the doctor needs to discontinue a patient’s Shockwave Therapy Package, a refund will be issued.
- Refunds will be calculated as follows:
- Remaining sessions will be refunded at a rate of $80 per session (except the final session).
- For the final session, the refund will be:
- $65 for the 3-visit package.
- $20 for the 6-visit package.
- Expiration:
- The 3-visit package expires 2 months from the date of purchase.
- The 6-visit package expires 4 months from the date of purchase.
- Sessions not used within the expiration period are non-refundable, and no credit will be issued after the expiration date.
- Clarification Rights:
- We reserve the right to provide clarification or explanation regarding the discount and refund process as needed.
